Affiliated providers and staff who participate in the CIN Collaborative on April 7, 2026 are essential to advancing high‑quality, cost‑effective care and excellence in patient experience within the UC San Diego Health Physician Network. As such, UC San Diego Health agrees to provide an honorarium to eligible participants or their practice.
Honoraria will be limited to a maximum of two participants per affiliated practice location, up to a maximum of 30 participants per medical group, at a rate of $150 per participant who meets attendance verification requirements. It is the sole discretion of UC San Diego Health and/or my medical group to determine which participants are eligible for the honorarium if attendance exceeds these limits. It is my responsibility to confirm which individuals will attend on behalf of my medical group.
Participants are expected to support the following activities:

– Attend a minimum of two (2) events or meetings in a given calendar year (note: members joining mid‑year are expected to attend the next scheduled meeting).
– Launch and oversee UC San Diego Health Physician Network–wide cost reduction and quality improvement initiatives.
– Advise on the data‑sharing requirements of the UC San Diego Health Physician Network as related to improving quality of care, care management and coordination, and patient access.
– Serve as a spokesperson for UC San Diego Health Physician Network quality activities within the affiliate’s medical group.

To receive the honorarium, participants must:

– Validate attendance by joining the meeting via Zoom using their unique registration link. Note that phone‑only participation does not qualify, as identity and participation cannot be verified.

– Submit a current W‑9 form, can be provided during registration or sent via email to PhysicianNetwork@health.ucsd.edu.
