Coming together to improve patient care
UC San Diego Health Physician Network is a collection of individual physicians, practices, and hospitals that have come together to improve patient care and reduce healthcare costs.
When did it start?
In 2014, UC San Diego Health began formally integrating with community physicians and local hospitals, collaborating on administrative functions, business, and clinical initiatives to improve efficiencies and benefit patients.
Today, UC San Diego Health is aligned with regional health systems and more than 900 select community physicians and specialists from San Diego, Imperial, Orange, and Riverside Counties. These providers, plus UC San Diego Health’s clinical faculty, which includes 150 primary care physicians, over 1,000 specialist physicians in 40 specialties, and hundreds of non-MD providers, are the foundation of our network.
Physician Network members collaborate on developing shared systems, infrastructure, care pathways, and quality initiatives to provide high-value healthcare to patients, employers, and health plans.
Why did we create the network?
Partnering with local physicians expands the geographic footprint for UC San Diego Health and increases collaborative care throughout the network. This collaboration makes our network more attractive to payers and large employer organizations we partner with to develop exclusive products.
How is it led?
The Physician Network has dedicated leaders and staff, and its own advisory board, including community physicians and UC San Diego Health faculty.

Duncan Campbell

Duncan Campbell, who has also served as chief operating officer of UC San Diego Health Medical Group since 2010, is a seasoned leader with more than a decade of financial and administrative experience in academic health systems. Campbell has been responsible for all financial and administration functions of the Medical Group, as well as directing the Managed Care Department.

Duncan Campbell, who has also served as chief operating officer of UC San Diego Health Medical Group since 2010, is a seasoned leader with more than a decade of financial and administrative experience in academic health systems. Campbell has been responsible for all financial and administration functions of the Medical Group, as well as directing the Managed Care Department.
He has developed physician affiliation strategies establishing new business models, overseeing the selection and implementation of revenue cycle systems and serving as a key member of the Board of Governors. He holds undergraduate and graduate degrees from Texas Tech University, with concentrations in finance and economics and health organization management.
Parag Agnihotri, MD

Parag Agnihotri, MD, is the Chief Medical Officer of UC San Diego Health Population Health Services Organization. Dr. Agnihotri provides executive leadership and guidance in developing the clinical components of our population health strategy across our clinically integrated network (CIN) and our Medicare Shared Savings Plan ACO.

Dr. Agnihotri focuses on transforming our clinical delivery network to adapt and thrive under new payment models such as global risk, capitated or accountable care models.
A few of Dr. Agnihotri priorities include assessing our managed care infrastructure, bringing on population health analytics for our MSSP ACO and global risk contracts, and educating and training the faculty regarding how to adapt clinical practices under a global capitation environment and partner with us on our payer strategies. He also coordinates activities across UC Health as the five UC Health campuses work together to build scalable population health tools.
Dr. Agnihotri is a faculty member in the division of geriatrics, and his clinical work focuses on clinical care oversight of our skilled nursing facility partners and outpatient care in the geriatrics clinic.
Prior to joining UC San Diego Health, Dr. Agnihotri served as the Medical Director of Population Health and Post-Acute Care for Sharp Rees-Stealy as well as the Chief Medical Officer for the Sharp Healthcare ACO. As a practicing geriatrician, he has published several articles on the management and outcomes of high-risk populations and is recognized as a national leader in the development of high-performing health systems, technology use, and population health management. Under his leadership, Sharp Rees-Stealy was been recognized as a “Top Performer” on HEDIS quality measures, designated “Elite Status” by the California Association of Physician Groups (CAPG), received high performance on the CAHPS survey, and was designated as a high performer from IHA on the Medicare Stars Program.
Before his time at Sharp, Dr. Agnihotri was the Medical Director of Care Management at Dignity Health’s Mercy Medical Group in Sacramento, where he achieved many of the same outcomes and dramatically improved the group’s HCC scores for the Medicare Risk Adjustment Factor (RAF).
Dr. Agnihotri completed his residency at the University of Illinois in Chicago and served as chief resident for the Department of Medicine before becoming the Division Director of Geriatrics at Michael Reese Hospital.

Loc Le is the chief financial officer of UC San Diego Health Population Services Organization, providing guidance and analysis for the clinically integrated network and Medicare Shared Saving Program Accountable Care Organization. He comes to UC San Diego Health from CalOptima and United Healthcare, where he served in financial and actuarial analysis leadership roles for many years.

Loc Le is the chief financial officer of UC San Diego Health Population Services Organization, providing guidance and analysis for the clinically integrated network and Medicare Shared Saving Program Accountable Care Organization. He comes to UC San Diego Health from CalOptima and United Healthcare where he served in financial and actuarial analysis leadership roles for many years.
Mr. Le has proven expertise in revenue improvement, provider reimbursement, cost management, budgeting, contract modeling, process improvement and performance analysis. He has a deep understanding of public and private health insurance, product pricing, provider contracting, risk analysis and claims. Mr. Le earned an MBA at California State University, Fullerton, and completed his BA in economics from UC Irvine.
