1) Is this a requirement?
A) Completing an annual census is a requirement of participating in the UC San Diego Health Physician Network. It is also a great way to ensure that we have all the providers enrolled with the necessary health plans.

2) Who/what do I include in this census?
A) On the first tab: please include physicians, nurse practitioners, physician assistants, and at least one lead administrator.
A) On the second tab: please include all clinic locations, phone and fax numbers, and fax addresses.

3) What if a provider only works remote/telehealth?
A) Please list the provider primary address as: Virtual

4) How do I submit my census?
A) Please follow the three steps above (download current census roster, update roster, submit your census).

5) Help! I have more questions.
A) If you require additional assistance or have any questions about the census, please email physiciannetwork@health.ucsd.edu.
