Affiliated providers and staff who participate in the CIN Collaborative on April 7, 2026 are essential for advancing high-quality, cost-effective care and excellence in patient experience within the UC San Diego Health Physician Network. As such, UC San Diego Health agrees to compensate these individuals or their practice. Compensation will be limited to a maximum of 2 participants per affiliated practice location, up to a maximum of 20 participants per medical group, at a rate of $150 per participant that attends the meeting. It is the sole discretion of UC San Diego Health and/or my medical group to determine which participants are compensated, should there be more than the limit set in this agreement. It is my responsibility to confirm who is attending within my medical group.

Participants are expected to support the following activities:

– Attend a minimum of two (2) events or meetings in a given calendar year (note: members that join mid-year will be expected to attend the next scheduled meeting).
– Launch and oversee UC San Diego Health Physician Network-wide cost reduction and quality improvement initiatives.
– Advise on the data sharing requirements of the UC San Diego Health Physician Network as related to improving quality of care, care management and coordination, and patient access.
– Serve as a spokesperson for the UC San Diego Health Physician Network quality activities within affiliate’s medical group.

In order to receive reimbursement, participants must (1) validate attendance of the meeting as required by UC San Diego Health, and (2) submit a current W-9 form.
